St. George’s University Hospitals NHS Foundation Trust is the regional specialist care provider for south west London, Surrey and beyond. The trust provides a high quality, comprehensive range of health services from leading edge tertiary and trauma care to community services, delivered from 2 hospital sites, St.George’s Hospital in Tooting and Queen Mary’s Hospital in Roehampton, and from St John’s Therapy Centre, 11 health centres and clinics, schools, nursing homes and people’s own homes. The trust also manages the healthcare service at HMP Wandsworth. With national and regional referrals added to our local 3.5m catchment, we serve a population of around 5m. Our End of Life Care strategy is founded on the Ambitions for Palliative and End of Life Care 2015-2020. Our vision is that ‘end of life care matters to everybody to ensure our patients die with choice and dignity’. To achieve this one key ambition in our strategy is that ‘our staff are prepared to care’. We want our patients and families to receive the best care by staff who are knowledgeable and skilful in end of life care.
We are working in partnership with Royal Trinity Hospice and primary care services in Wandsworth to create an education and development strategy and learning programme to support the implementation of our End of Life strategy.
We have been successful in gaining support from Health Education England and would like to appoint a Project Lead and a team End of Life Care Facilitators to help make our aspirations a reality.
