This is an opportunity to join the Children’s Community Nursing Service which provides care for children and young people aged 0 – 18 years who are registered with a general practitioner in Northumberland and North Tyneside.
The post holder will work with children and young people 0 – 18 years with life shortening conditions and complex health needs to support them in accessing a variety of short break care opportunities in their community. This would include leisure facilities and community groups.
The post holder will work on a one to one basis following care plans, initiated and supported by the Children’s Community Nurses, to provide care and support that is flexible and responsive to the individual health needs of a particular child or young person. They will also be required to work in partnership with the children, their families / carers and with other professionals / agencies in delivering support and care to meet the child and young persons assessed health needs.
Training will be provided to support the post holder. The post holder will support Consultant Paediatricians and other medical staff in locality clinics.
Essential Criteria
Candidates must be able to demonstrate the English language proficiency level required for this post.
Due to the nature of this role, it is an essential requirement of the role that the post holder has a valid driving licence and is either a car owner and able to use the car for work purposes, or has a Trust personal lease vehicle which may be used for the role. However, please note that the Trust welcomes applications from disabled people whatever their impairment and would consider making reasonable adjustments to the role, if necessary, to enable a disabled person to undertake the role.
The post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).
