The Manchester CCG CHC team is a dynamic and innovative place to work. We have invested additional resource into the CHC team and are implementing a new model of care built around structured workflows. We are entering an exciting period of change and we are seeking motivated, patient focused people to join the team.
The post holder will be responsible for gathering information to enable a referral to be input onto the database. You will be a primary point of contact for internal employees and external stakeholders. Sending letters, booking meeting and assessments will be part of the day to day role. In addition, regular access to the database and management of the Broadcare admin workflow.
The CCG is now using the Broadcare database, part of Brayleino and has been in operation for almost 2 years. The post holder will assume responsibility for the inputting of patient information onto the database and affiliated operational procedures and processes linked to Continuing Healthcare (CHC) and Funded Nursing Care (FNC).
You will have the support of CHC management as well as the support of the wider CHC team. The continued skill set development of CHC, FNC, JWA and Fast Tracks will be key to the role and will involve a high level of attention to detail and possible infrequent travel between Abbey Hey Clinic and Parkway Business Centre. Candidates should be used to working in a fast paced and timescale driven environment. The post holder will responsible to the Business Manager
