Who we are
Hounslow & Richmond Community Healthcare NHS Trust (HRCH) is responsible for providing community health service for over 500,000 people living across the London Boroughs of Hounslow and Richmond-Upon-Thames.
Community Healthcare covers most of the services that are provided outside of hospital or GP surgeries. We are passionate about delivering high quality care and provide a range of services for both adults and children including community nursing, health visiting, physiotherapy, nutrition and dietetics, health promotion and stop smoking services, speech and language therapy and occupational therapy. We also provide some specialist services such as audiology, neuro-rehabilitation, continence services and continuing care.
Some of our services such as the Walk-in Centre at Teddington Memorial Hospital and the Hounslow Urgent Care Centre attract patients from nearby boroughs and we also run services further afield such as the LiveWell and stop smoking services in the London boroughs of Sutton and Merton.

Primary Care Patient Co-ordination Service – Hounslow
Hounslow Clinical Commissioning Group (HCCG) is made up of 49 practices which comprise of 5 localities Great West Road, Feltham, Chiswick, Brentford and Isleworth and Heart of Hounslow. There is a registered population of 303,000 patients and a health budget of over £308 million.
The CCG’s vision is to improve the care provided to patients, reduce health inequalities and raise the quality and standards of GP practice while achieving financial balance.
HRCH are looking to recruit 2 whole time equivalent posts to work in this growing service, proactively supporting the Hounslow GP practices.
The primary objectives of the Patient Care Coordination Service will be to support general practice to effectively manage its patient population. Ensuring the care delivered by general practice is more co-ordinated, proactive and planned.
These posts will support the delivery of coordinated, proactive and planned care for patients registered with Hounslow CCG Practices.
The service provide a team of coordinating staff to enhance and improve pro-active care, prevention and self-care for patients including at risk groups, over 65’s, patients with long term conditions and those receiving health and social care support from multiple providers.
The post holder will provide a comprehensive support service carrying out general administrative duties include, signposting, information sharing and data collection to ensure the smooth running of the Primary Care Patient Coordination Service.
The service is provided to all 49 Hounslow CCG GP Practices in the five localities therefore travel will be essential as the Band 4 staff will be deployed from the centralised hub out to practices, on a regular basis in order to support the delivery of this new and exciting contract.
